Health Forms and Insurance

It's important that your student complete the necessary medical forms prior to summer orientation. In addition, your student must make some decisions about their health insurance. (We have a feeling some of them may ask for your help with that one.)

Below, we've outlined the steps they'll need to follow.

Forms

Each incoming first-year, transfer, graduate, doctoral student, or visiting scholar must complete the following forms by July

Insurance

Enrollment in Springfield College student health insurance is mandatory for all students. Your student may waive out of the student health insurance and the related fee if your student has proof of comparable health insurance coverage. 

The insurance waiver is due in September, but your student is strongly encouraged to complete it as soon as possible.

Physical Examinations

All new students must have had a physical examination one year prior to the first day of classes. If your student is not yet due for your insurance-covered annual physical, they are welcome to schedule a physical exam appointment free of charge at the Health Center.
