Each incoming first-year, transfer, graduate, doctoral student, or visiting scholar must complete the following forms, which can be found here. The deadline for health forms is July 15, and the insurance waiver form deadline is Sept. 15.

Insurance
Enrollment in Springfield College student health insurance is mandatory for all students. You may waive out of the student health insurance and the related fee if you have proof of comparable health insurance coverage.
The insurance waiver is due Sept. 15, 2019, but we strongly encourage you to complete it as soon as possible.

Physical Examinations
All new students must have had a physical examination on or after Sept. 5, 2018, which is one year prior to the first day of classes. If you are not yet due for your insurance-covered annual physical, you're welcome to schedule a physical exam appointment free of charge at the Health Center.
