To qualify for disability services and resources, students are required to provide current documentation from the appropriate licensed diagnostician familiar with the history and functional limitations of the impairment (see specific guidelines below). Failure to provide all required information may prevent the student from receiving requested accommodations and/or services.

  • A description of the expected progression of stability of the disability
  • A description of the current accommodations, services or modifications
  • Recommendations for accommodations, adaptive services, assistive services, compensatory services, and/or collateral support services
