Forms at Camp Massasoit
Please print off the Camp Massasoit Parent Information Packet, which provides additional information on Camp Massasoit, as well information regarding all forms, refund policies, late fees, camper pick-up policies, and billing information.
Below are the remaining forms that are required to complete the registration process.

Emergency Contact & Health Record Form — Must complete
Medication Authorization — This form only needs to be filled out if your child needs to take medication during his or her time at camp.
Also make sure to include a copy of your child's most current immunization records, as well as their most recent physical exam when submitting your paperwork. All forms must be on file prior to your camper starting at Camp Massasoit.

Authorized Pick-Up List
If you need to update your pick-up list, you can do so by logging into your child’s account, under account members click your child’s name and then scroll down to Custom Forms and then select edit.
