Information and forms for the 2021 camp season will be available soon.
Registration will open on Wednesday, April 21 at 9:00 a.m. for our returning families. If there are slots still available, we will open up to the general public on Friday, April 23 at 9:00 a.m..
Please print off the Camp Massasoit Parent Information Packet, which provides additional information on Camp Massasoit, as well information regarding all forms, refund policies, late fees, camper pick-up policies, and billing information.
Below are the remaining forms that are required to complete the registration process.
- Emergency Contact & Health Record Form (PFD) — Must complete
- Medication Authorization (PDF) — This form only needs to be filled out if your child needs to take medication during his or her time at camp.
- Camper Release Form (PDF) — This form only needs to be filled out if you need to add someone to your child's pick-up list.
Also make sure to include a copy of your child's most current immunization records, as well as their most recent physical exam when submitting your paperwork. All forms must be on file prior to your camper starting at Camp Massasoit.
